How Do I Become a Patient?

If you would like to become a patient at Centennial Pediatrics of Spokane, we would first recommend looking at our website and the "Policies" page to make sure our policies and values align with yours. If so, please call our office to get registered.


During the registration process, our staff will walk you through which new patient paperwork needs to be completed and answer any questions you may have. 


NEW PATIENTS FROM OUT-OF-STATE:

If your family has relocated to Spokane from out-of-state, please bring vaccine records to your child's first visit so that we can review them at that time. 


Obtaining Medical Records

We ask that an adult familiar with the child's medical history come to the first visit so that we can make sure we have an accurate picture of the child's health. We will have you complete a Medical Records Release at the first visit or you can complete this ahead of time. However, we cannot guarantee that the records will have been received by your child's first visit as this relies on the releasing office.
